What should I bring to my first appointment?

Please bring a valid photo ID, insurance card, current medication list, relevant medical records, and any recent lab or imaging results if available.

How often should I schedule an annual physical?

Most patients benefit from a yearly wellness or preventive visit. Frequency may vary depending on your age, medical history, and chronic conditions.

Do you offer same-day appointments?

Yes, same-day appointments may be available for urgent medical concerns such as illness, infections, or other non-emergency issues. Availability may vary.

Do you offer telehealth visits?

Yes. Telehealth appointments are available for many follow-up visits, medication management, lab review, chronic disease management, and select urgent concerns.

How do I request prescription refills?

Prescription refill requests can be submitted by contacting the office or through your pharmacy. Please allow 48–72 business hours for refill processing.

How will I receive my lab or imaging results?

Results are reviewed carefully and communicated through follow-up appointments, telehealth visits, phone calls, or the patient portal depending on the situation.

How long does paperwork take to complete?

Please allow 5–7 business days for forms, disability paperwork, FMLA, and other medical documents. Additional fees may apply for certain forms.

What is your cancellation policy?

We kindly ask for at least 24 business hours’ notice for cancellations or appointment changes. Missed appointments or late cancellations may be subject to a fee.

What ages do you see?

Do you treat chronic medical conditions?

Yes. We provide ongoing management for conditions such as diabetes, hypertension, high cholesterol, thyroid disease, heart disease, obesity, and more.

What if I have an urgent medical issue after hours?

If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room. For urgent concerns after hours, please contact the office for guidance.
